The question of whether a provider is appropriate for ceremony has nothing to do with the aesthetic — the setting, the amenities, the framing — and everything to do with whether they screen for cardiac conditions, require an on-site physician during the 12–24 hour active ceremony, and maintain the monitoring infrastructure that ibogaine actually requires.

Wellness is a positioning choice. It signals that a facility wants to appeal to people seeking retreat-style experiences rather than clinical ones. The category is not regulated. There is no credential required to call an iboga programme a wellness centre, and the label carries no implication about what the facility actually does when something goes wrong during a 12–24 hour active ibogaine ceremony.

The iboga market has expanded significantly over the past decade, and so has the range of what providers call themselves. Some facilities with strong medical infrastructure use the wellness framing because it attracts a broader audience than clinical language does. Others use it specifically to avoid medical connotations while offering programmes without the associated requirements. The label itself does not tell you which you are looking at.

What you are evaluating when you assess an iboga facility is not the category it occupies but the specific infrastructure it maintains. Red flags to identify before booking

The following are signals that a facility is not providing adequate infrastructure, regardless of how the programme is presented:

Pricing significantly below $2,000 CAD for iboga ceremony.This is not a bargain. It is a budget. The cost has gone somewhere — typically the physician, the monitoring equipment, or both. Providers who operate below market rate are not being generous. They are reducing safety infrastructure.

No mention of a physician or medical staff. Facilitators, coaches, and integration specialists are not equivalent to medical professionals during the active ceremony. A website that describes the facilitation team without referencing medical personnel is describing a non-medical programme.

No medical screening requirement before booking. A provider that does not require a baseline ECG and medical history review before ceremony is not screening for cardiac contraindications. Who is not a candidate

These are absolute contraindications for iboga ceremony, independent of how good a provider is or how strong the setting appears:

Significant cardiac conditions including QT prolongation. Ibogaine extends the QT interval. People with pre-existing QT prolongation are at elevated risk for potentially fatal arrhythmias. This is the contraindication that makes cardiac screening non-negotiable.

Current SSRIs or SNRIs without a completed supervised taper.The combination creates conditions for serotonin syndrome, which can be fatal. No exceptions. Pregnancy. Absolute contraindication.

Acute psychiatric crisis.Someone in acute crisis is not an appropriate candidate at this time, regardless of how urgently they feel the need for something different. The medicine amplifies what is present. Entering a 12–24 hour ceremony in a state of acute destabilisation does not produce stabilisation — and saying so directly, even when the person does not want to hear it, is what honest medical screening does.

A provider that does not screen for these conditions is not reducing barriers to access. They are transferring risk to the person in ceremony.
