How Do You Take Ibogaine? Dose, Method, and What It Requires

Ibogaine is taken orally — capsules of ibogaine hydrochloride, calculated at 15–20 mg/kg of body weight. The oral route is not a choice made for convenience. Ibogaine cannot be vaporised, smoked, or injected safely with its current risk profile; the oral dose, timed and monitored, is the only administration route used in legitimate clinical and ceremonial contexts.

What makes this medically significant is not the capsule. It is what must be in place when the capsule is given: a physician on-site throughout the full 12–24 hour active experience, continuous cardiac ECG monitoring, and a protocol that cannot be shortened because the person is uncomfortable.

What Form Ibogaine Comes In

Ibogaine is administered in one of two forms: ibogaine hydrochloride (HCl) or total alkaloid extract (TA). Ibogaine HCl is the isolated compound — a white crystalline powder, encapsulated and dosed by body weight. TA extract contains ibogaine alongside other alkaloids from Tabernanthe iboga root bark and produces a slightly different experiential quality, though the core pharmacology is similar.

The third option — whole root bark — is used in traditional Bwiti ceremony. The root bark contains ibogaine and noribogaine alongside dozens of other alkaloids. Dosing whole root bark by precise body weight is more difficult than dosing isolated HCl; traditional contexts use experiential markers rather than measured milligrams. Legitimate ceremonial providers who use root bark in a traditional Bwiti framework are doing something meaningfully different from a clinical ibogaine protocol, even when the active compound is the same.

For the purposes of how ibogaine is administered medically — which is what most people searching this question are asking — ibogaine HCl in capsule form is the standard. It is what allows precise dosing by body weight and what clinical research uses.

How the Dose Is Calculated

The standard flood dose is 15–20 mg/kg of body weight. At 70 kg, that is 1,050–1,400 mg of ibogaine HCl. This is not a dose to estimate. The difference between 14 mg/kg and 20 mg/kg is significant — in both experience and cardiac risk.

Providers who dose by body weight and verify via pre-ceremony EKG are doing it correctly. Providers who name a flat dose — "we give everyone 1,000 mg" — are either not adjusting for body weight or are describing their most common dose without clarifying that they individualise. Ask directly.

The dose may be adjusted downward for individuals with certain health parameters, lower body weight, or prior ibogaine exposure. Upward adjustment beyond 20 mg/kg is rare and adds cardiac risk without proportionally increasing benefit in most cases. The goal is not a larger dose — it is the appropriate dose for the specific person.

The Test Dose — What Happens First

Before the full flood dose, most legitimate providers give a test dose — typically 1–3 mg/kg, administered 2–3 hours before the main dose. The test dose serves two functions.

First, it allows the physician to observe how this specific person's cardiac conduction responds to ibogaine before administering the full amount. Even with a clean pre-ceremony EKG, some individuals show prolonged QT intervals under ibogaine. The test dose catches this before it becomes a crisis at full dose.

Second, it gives the participant an initial encounter with the medicine — a lower-intensity experience that provides some orientation before the full experience begins. Most people notice mild visuals, ataxia, and an altered sense of time within the first 45–90 minutes.

If the EKG during the test dose shows a concerning change, the full dose does not proceed. This is not a formality. It is the built-in safety checkpoint that separates a medically supervised ibogaine administration from a dangerous one.

What Monitoring Must Be in Place

Ibogaine prolongs the QT interval — the cardiac electrical cycle that, when disrupted, can produce fatal arrhythmia. This is not a theoretical risk. Ibogaine-related deaths are almost uniformly associated with undetected QT prolongation, drug interactions, or an absent physician.

Continuous cardiac ECG monitoring is required throughout the active experience — from the test dose through the resolution of the full dose, which typically means 12–24 hours. A physician must be physically present on-site, not available by phone. The distinction matters: if a cardiac event occurs at hour 14, the intervention window is minutes.

Any ibogaine provider who skips cardiac screening is not operating safely. The EKG is how you identify QT prolongation before ceremony, not during. This is not optional infrastructure — it is the only reason that ibogaine, with its real cardiac risk profile, can be administered responsibly. A provider who suggests the EKG is a formality or that their experience allows them to skip it is telling you exactly how they have cut corners on previous participants.

The 2023 Stanford study published in Nature Medicine — 30 special operations veterans, 88% reduction in PTSD symptoms at one month post-treatment — was conducted with full cardiac monitoring throughout. That is not incidental. It is what made the study protocol safe enough to publish.

What Happens in Your Body After You Take Ibogaine

Ibogaine is metabolised to noribogaine, which has a significantly longer half-life. The active pharmacological experience lasts 12–24 hours. Noribogaine, however, remains biologically active in the body for weeks to months after a single flood dose.

This extended activity is part of what makes ibogaine different from other interventions. The acute experience — which is challenging, disorienting, and not recreational — is followed by a period of elevated neuroplasticity during which integration work has an outsized effect on whether any lasting change results. The 4–8 weeks after ceremony are not a wind-down period. They are when the medicine's effects on habit formation and mood regulation are most accessible.

Recovery takes 2–3 days post-ceremony. Most people spend those days resting, with support, at the facility — not returning immediately to their regular lives. Ataxia persists for 24–48 hours after the full dose in most participants. Full sensory and cognitive clarity typically returns by day 3.

Who Cannot Take Ibogaine

This section is not a footnote. It is the most important part of the page.

Absolute contraindications — conditions under which ibogaine cannot be taken safely at any provider or at any price:

  • QT prolongation or significant cardiac arrhythmia on pre-ceremony EKG
  • Current SSRIs or SNRIs without a completed, physician-supervised taper (serotonin syndrome risk is real and potentially fatal)
  • Methadone without a completed transition protocol to a shorter-acting opioid
  • Lithium and most antipsychotic medications
  • Active psychosis or schizophrenia-spectrum disorder
  • Severe liver or kidney disease (ibogaine is hepatically metabolised)
  • Pregnancy
  • Recent myocardial infarction

These are not cautions to weigh against potential benefit. They are conditions under which ibogaine is not administered at a legitimate provider. Any provider who screens you and confirms a ceremony date despite one of these applies has made a decision about your risk that you should not accept on their behalf.
